Short-term rental property agents should use this form when accepting the role of responsible agent on behalf of the property owner. NOTE: a separate form is required for each rental unit. Download the fillable form. Fill out the form and upload to your Certificate of Use application in Pinellas County Access Portal. If you already have a Certificate of Use, upload the form to the Certificate of Use record in the Pinellas County Access Portal. This form must be accompanied by the Owner Affidavit form.
