Short-term rental property owners must complete this form if an agent is to be assigned on behalf of the property owner. Note: a separate form is required for each rental unit. Fill out the form and upload to your Certificate of Use application in Pinellas County Access Portal. If you already have a Certificate of Use, upload the form to the Certificate of Use record in the Pinellas County Access Portal. This form must be accompanied by the Agent Affidavit form.
