Personnel File Request Procedure
You can request personnel files using any of the following options:
- Form: Complete and submit Personnel File Request Form.
- Mail: Send your request to Human Resources, 400 South Fort Harrison Avenue, Clearwater, FL 33756.
- Phone: Call Human Resources at (727) 464-3367.
The requester is responsible for any cost of providing the documentation, which includes staff time, cost of copies, and other costs that are associated with the request. Files are provided in electronic/digital format (PDF) unless you request a paper copy. If you request paper copies, payment by check must be received prior to copying. The charge is $0.15 per page for one-sided copies and $0.20 per page for two-sided. Please make the check payable to Board of County Commissioners.
Information that is protected under public records law, such as Social Security numbers, will be redacted (removed) before a file is provided.
Personnel files will be provided in a reasonable timeframe based on the scope of the records request.
