Instructions for property agent: Use this form when accepting the role of responsible party for a short-term rental unit on behalf of the property owner. If completing as an individual, and you are not part of a property management or other company for this role, enter N/A in the business sections that do not apply. This completed form and an Owner Acknowledgment form must be submitted together online. NOTE: A separate form is required for each rental unit.
