The biennial Employee Voice Survey will be open to employees from August 18 to September 8, 2025. The questions, which are the same as the previous 2023 survey, are shown below.

The sample below matches the actual survey except the formatting. For example, there are headings below to illustrate the different categories, and the answer choices are listed in italics after the question rather than in columns with fillable circles to mark for each item.
