Waste Collection Assistance Program Request

Residents with a disability that prohibits them from being able to place their waste at the roadside for collection are eligible to apply for this program.

Directions:

  • Please complete the top portion of this form.
  • Print the form and sign and date it. Make sure you have read and understand the narrative.
  • Send the completed form, along with the required doctor’s certification, to the address at the top of the form.

Mobile Home Park Exemption Request

  • To obtain a copy of this form, please call (727) 464-7500 and ask to speak to the Lealman Coordinator. Directions on how to complete the form can also be provided.

Vacant Residential Unit Refund Request

Directions:

  • Please complete sections 1 – 6 (Vacancy Period, Owner’s Name, Telephone Number, Owner’s Address, Tax Parcel ID Number, Parcel Address) of this form.
  • Provide documentation for sections 7 – 9. You can verify payment of the assessment via the Tax Collector’s database. Copies of the electric bills for the year can meet the requirements of sections 8 and 9.
  • Sign the form and have it notarized. Send the completed form, along with the required documentation, to the address on the form.

Note: The form must be post marked by March 31st of the following year for which the refund is sought. Failure to submit the form on time is deemed a waiver of the right to a refund.
