Good health is key to a fulfilling life. Annual screenings help monitor your risk of heart disease, diabetes, cancer, and other serious health conditions.
Employees who complete an annual biometric screening and clinical health risk assessment (CHRA) by November 30, 2026 earn a preferred medical plan premium which will save $500 in 2027.
- Biometric Screening (Quest): Visit My.QuestForHealth.com to make an appointment for a Direct-to-Quest Diagnostics® Patient Service Center or an onsite screening. Access the Physician Lab Form on UMR’s Live Well Reward$ platform (log into UMR.com, select Health Center, and Live Well Reward$).

- Employees whose medical coverage begins on July 1, 2026 or later
- Employees who opt out of the County’s medical plan

Why should I do the biometric screening and clinical health risk assessment (CHRA) each year?
The goal is to identify your health risks, such as high blood pressure or diabetes, which may be prevented through early detection and lifestyle changes. We recommend reviewing your results with your doctor.

How do I qualify for the 2027 preferred premium and save $500?
- Complete a biometric screening between January 1 and November 30, 2026. The biometric screening uses bloodwork and body measurements to identify potential health risks.
- Quest Login: Existing users can log in with their credentials set up in 2025. New users need to set up a new My.QuestForHealth.com account using the registration key Pinellas2026, then create a new username and password.

- Option 1: Direct-to-Quest Patient Service Center Appointments – Use this quick, convenient and paperless option. It’s always free even if you have other lab work ordered by your doctor.
- Option 2: Onsite Biometric Screenings (coming soon!) – For your convenience, Quest will provide screenings at various worksites.
- Option 3: Physician Lab Form – Use for in-network lab work ordered by your doctor (i.e., BayCare, Labcorp, etc.), or for convenience care clinics (Walgreens, CVS, etc.). Paperwork is required. If you use the physician form, you do NOT need to set up an account on MyQuest. Physician forms are downloaded through UMR’s Live Well Reward$ platform (log into UMR.com, select Health Center, and Live Well Reward$). Please upload the form as early as possible since it can take up to 2 weeks for the form to be processed.
- Complete the clinical health risk assessment (CHRA) by November 30, 2026. This online questionnaire collects information about your current health status and conditions, including lifestyle behaviors, health-related activities, and adherence with recommended preventive screenings.
- Take the CHRA on UMR’s Live Well Reward$ platform (log into UMR.com, select Health Center, Live Well Reward$, and Clinical Health Risk Assessment.
- Take the assessment as soon as possible.
- You do not have to wait for the biometric screening lab results to populate in your assessment.
Which biometric screening option should I use?
In order for your results to be counted as complete, you must use Option 1: Direct-to-Quest, Option 2: Onsite Biometric Screenings (coming soon), or Option 3: Physician Lab Form.
- Direct-to-Quest and onsite screenings are the quickest, most efficient ways to get a biometric screening.
- If you want to use do Direct-to-Quest and additional labs your doctor orders, you must make an appointment using the Pinellas Quest site AND a separate appointment through the regular Questdiagnostics.com site for your doctor’s lab orders. That way, the lab technician at Quest knows they need to draw blood for separate lab work.
- If you want your doctor to order all of the lab work, including everything on the biometric screening, use the Physician Lab Form.
- Do NOT make an appointment on the Pinellas Quest site.
- Make an appointment at any in-network lab.
- We encourage you to complete the lab work your doctor ordered and have the doctor complete the form early in the year since it can take up to 2 weeks for the form to be processed.
- Upload the form as early as possible to UMR’s Live Well Reward$ platform due to the longer processing time.
- Another option is to print the Physician Lab Form and take it to a convenience care clinic such as CVS or Walgreens for completion and signature.
What items are measured in the biometric screening?
- Blood pressure
- Height
- Weight
- Waist circumference
- Cholesterol (HDL, LDL, total)
- Triglycerides
- Glucose (blood sugar)
- A1C (optional test to measure your average blood sugar level, available with or without a diabetes diagnosis)
If my annual physical is in November or December, what should I do?
Get your screening done early at one of the onsite screening locations throughout the year (coming soon!). If you are not able to attend, using Direct-to-Quest is highly encouraged because it’s quick, paperless, and results are available in 2-3 days.
However, if you prefer to have your doctor order the lab work, tell your doctor you do NOT have to wait 365 days between your annual physicals because our medical plan runs on a calendar year basis which means insurance will cover the cost of the visit and preventive lab work any time during the year. Schedule your appointment earlier in the year if you use the Physician Lab Form and have your doctor order lab work as part of your annual physical. For example, if your last physical was in November, you can schedule the appointment earlier without incurring any cost.
If your doctor insists you wait 365 days before scheduling this year’s appointment, use the Direct-to-Quest option to ensure you meet the November 30 deadline.
What questions are on the health assessment (CHRA)?
The CHRA takes approximately 15 minutes to complete. It gathers important data about daily life and health and provides context for real-life challenges people may face that could impact their health. This data helps us to design wellness programs. For example, if we learn that 75% of our employees have trouble getting enough sleep, we could offer sleep-related resources. The assessment also helps UMR offer programs to assist employees with their needs. For example, someone who reports that they are struggling with high cholesterol may receive helpful information from UMR’s GenYou team related to cholesterol management.
This personalized outreach is not shared with Pinellas County, only aggregate data such as 40% of employees have high cholesterol. UMR provides only overall trend data to Pinellas County that cannot be tied to any individuals. For questions, contact the Wellness team at wellness@pinellas.gov or (727) 464-3367, option 1.
What if I got bloodwork done but did not use the Direct-to-Quest, onsite screening, or Physician Lab Form options?
Your biometric screening will not be considered completed unless you use Direct-to-Quest, onsite biometric screenings, or the Physician Lab Form.
Schedule another blood draw using the Direct-to-Quest Patient Service Center appointment scheduler at My.QuestForHealth.com (no additional cost even if you already had lab work done).
How long does it take for my biometric screening to show as completed in UMR’s Live Well Reward$?
It takes approximately 2 to 3 weeks to see your Quest biometric completion in Live Well Reward$. It takes an additional week for your lab results to populate into the clinical health risk assessment (CHRA). We recommend you do not wait for your results to populate to take the assessment.

Do I need to set up a new account on My.QuestForHealth.com?
Yes, but only if you choose Direct-to-Quest or an onsite screening, and you did not participate in Quest screenings last year. The Physician Lab Form can be downloaded from UMR’s Live Well Reward$ Wellness portal. Set up your new My.QuestForHealth.com account using the key Pinellas2026 on the green side of the login page. You will need to create a new username and password.
How does the $500 premium discount work?
Employees who successfully complete an annual biometric screening and clinical health risk assessment (CHRA) by November 30, 2026 earn a preferred medical plan premium which will save $500 in 2027. If you do not complete both activities by the deadline, the non-reduced portion of your medical premium totaling $500 will be deducted from your paycheck in the amount of $29.42 per pay period beginning with the first May 2027 paycheck and will be divided over the remaining pay dates in 2027.
Do I need to use leave time?
You may get an onsite biometric screening during work time with prior approval from your supervisor if the location and timeframe are reasonable and do not interfere with the department’s ability to continue County services. For an offsite biometric screening, you will need to use approved leave.
