Type 2 Diabetes Improvement Program

Pinellas County employees and their adult dependents on the County’s medical plan now have access to the Level2 Specialty Care by UnitedHealthcare. This is your opportunity to not just manage type 2 diabetes, but work to improve it, with support and tools including an expert care team.

This unique program offers a comprehensive approach to glucose control based on criteria from the American Diabetes Association. Benefits include a free continuous glucose monitor providing 24/7 data collection of your glucose levels — all without daily fingersticks!

What will I get?

If eligible, you receive the following at no cost:

  • A continuous glucose monitor (biosensor applied to your arm) that pairs with an app on your smartphone, estimated value $2,500/year
  • Virtual access to an expert care team of providers, nurses, dieticians, and coaches to help guide you
  • Dedicated app with resources including videos

After you sign up, you’ll receive a start-up kit in the mail, meet virtually with a specialist to set up your glucose monitor, and schedule your first meeting with the care team.

Who is eligible?

The program is open to employees and their adult dependents who are on the Pinellas County medical plan (PPO or HDHP) and have type 2 diabetes.

How much does the program cost?

There is no cost to you.

Can I earn Wellness Incentive Program rewards for participating?

No. But you can earn Wellness Incentive Program rewards of $100 or 25,000 points for participating in the Ongoing Condition CARE program for diabetes.
