Affidavit of Domestic Partnership and Certification for Dependent Tax Status

2027 Update

The 2027 Affidavit of Domestic Partnership Form will be provided in the Workday platform. Those enrolling a domestic partner for 2027 benefits during Open Enrollment will complete a non-notarized digital affidavit and upload supporting documentation in Workday. If needed, complete the 2027 Certification for Dependent Tax Status Form.
