Pinellas County’s goal is to increase access to quality health care, improve health of low-income/high-risk individuals and reduce health inequity. Health Care Services, a division of Human Services, partners with the Florida Department of Health in Pinellas County to provide prevention-focused health care.

What is a Medical Home?

A Medical Home is your first stop for health care. A Medical Home is the place you can go for primary care and to help you receive other care offered in the Pinellas County Health Program. In addition, your Medical Home provides easy access to medical staff through same-day office appointments and physicians who are on call 24 hours a day, 7 days a week for emergencies. How Do I Sign Up for the Health Program?

To sign up for the Pinellas County Health Program, you must complete an online application from any computer with an internet connection.

Accomplishments

Family Housing Assistance Program
  • 64 families housed during FY22/23
    • An Increase of 23% from FY21/22 in which 52 families were housed.
  • The 64 families who were housed, were housed in an average of 131 days.
    • 30% improvement from the last FY where it took an average of 187 days.
  • 79% (53 out of 67) of families exiting FHAP successfully completed the program.
  • 27 Adults entered and engaged in FHAP Workforce Development during FY22/23
    • 67% (10 out of 15) of adults exiting FHAP Workforce Development services increased their income.
    • The average monthly increase in HH income for these 10 families is $1,089.56.
Community Outreach/Community Services Coordination
  • 24 unduplicated presentations to increase awareness about the County Health Program services, application process, and eligibility requirements.
  • Implemented quarterly outreach schedule to community partners.
    • 673 contacts completed YTD
Eligibility Initiatives
  • Implemented a formal process to assist clients with scheduling their initial primary care appointment during enrollment.
    • 855 medical home appointments were successfully scheduled.
  • Implemented a formal Direct Entry/Phone Application Process to assist clients who have barriers utilizing pre-existing application methods (technology, reading/comprehension, etc.). This process also assists in mitigating abandoned E-Apps
    • 882 Direct Entry Applications Completed (92 % approval)
    • 241 Phone Applications Completed (73% approval)
  • Processed 695 medical records requests.
  • Processed 79 HCRA Cases and one (1) successful Appeal upheld.
  • Implemented a formal process to track referrals submitted to the ACA Navigator
  • 89 referrals made between 4/1 – 9/30
