Opt Out Incentive Summary and Affidavit
2027 Update
The 2027 Opt Out Incentive Summary and Affidavit Form will be provided in the Workday platform. Those who want to opt out of medical plan benefits in 2027 during Open Enrollment will complete a non-notarized digital affidavit found in your Workday My Tasks inbox after submitting your enrollment.
Please review the affidavit carefully. If any part is not true for you, do not submit. Instead, go back through your enrollment elections and change your medical election to waive Opt Out. If you waive all of the choices, including Opt Out, you will be declining all medical coverage.

Eligibility
To qualify for the opt out incentive, you must NOT be:
- An elected official, per Florida Statutes Chapter 145
- Covered by a plan sponsored by the Pinellas County Board of County Commissioners including the Pinellas County Sheriff’s Office
- Covered by a plan that does not meet the affordability requirements and minimum value as defined by the Patient Protection and Affordable Care Act of 2010 (“ACA”)
- Covered by a plan that is an individual healthcare marketplace plan (Healthcare.gov) or any medical benefit plan not established or maintained by an employer, an employee organization (such as a union) or both
- Covered by Medicare, Medicaid, or any plan funded in whole or in part by the Pinellas County Board of County Commissioners
