Aetna’s Healthy Home Visits Program

Good health starts in the home. Aetna® Healthy Home Visits gives you access to preventive care at no added cost — it’s already included in your plan. This allows you to get an annual visit (in-home or virtual) from a licensed doctor or nurse to assess your health and safety needs. It does not replace your regular healthcare visits, nor does it change your healthcare plan or benefits.

During the visit, a Signify Health clinician will do the following:

• Check your vitals
• Review your medications
• Check for safety risks
• Discuss your medical history
• Answer any health questions you may have

Afterwards, the clinician will share a visit summary with your doctor to keep them in the loop. Family members may be present for the visit. To schedule a visit, call (855) 746-8709 or use the online scheduler.
